Students who need a form completed by the Office of the Registrar—such as an in-school deferment, scholarship, licensure, internship, or a parent–provided insurance form—may submit their form to our office for completion. Students may also request verification letters with information that is not available on the self-service verification letter.

Please allow 3–5 business days for us to complete your request.
Where We Send Verifications Once Completed
All verification requests will be emailed directly back to you at your UC email address. If you have a special circumstance requiring a physical form be completed in ink, please email us at [email protected] to coordinate.
What We Can Verify
We are only able to verify information that is officially on your academic record. This includes data such as your academic standing, enrollment status, and earned degree(s). If you are not registered—or your degree has not yet been certified—we will not be able to complete your request. Check your enrollment or degree status in Catalyst prior to submitting your request.
How to Submit Your Form for Completion
- Prepare your form
- Fill out your information (i.e., any sections on the form that are required to be filled out by the student, parent, and/or borrower)
- Leave any Office of the Registrar sections blank (i.e., any school certification/Registrar verification sections)
- Submit the Form Completion Request below
